DURING BUSINESS HOURS

Appointment Scheduling

  • Patients are encouraged to schedule appointments in advance.
  • Walk-in patients will be seen as time permits.
  • Cancellations should be made at least 24 hours in advance. Failure to do so may result in a cancellation fee of $50.

Patient Arrival and Wait Times:

  • Patients should arrive at least 15 minutes prior to their scheduled appointment time to complete any necessary paperwork.
  • Wait times may vary depending on the nature of the appointment and emergencies. We aim to minimize delays.

Payments:

  • Cash Only: All payments for visits, treatments, and form completions must be made in cash at the time of service.
  • No Credit or Debit Cards Accepted.

Medical Records and Privacy:

  • All patient information is confidential and protected by HIPAA regulations.
  • Requests for medical records should be submitted in writing, and a processing fee may apply.

Office Behavior

  • Patients are expected to treat all clinic staff with respect and courtesy.
  • Disruptive behavior, abuse, threats or harassment will not be tolerated and may result in dismissal from the clinic.

Emergency Services

  • This clinic is not an emergency facility. In case of a medical emergency, please call 911 or go to the nearest emergency room.
  • Our office will provide care during office hours, but after-hours care is not available.

Follow-Up on Reports and Referrals:

  • It is the patient’s responsibility to follow up with the specialist’s office and keep track of referrals.
  • Ensure you follow up on any reports, test results, or medical recommendations provided by the clinic.
